With my signature below, I confirm that I have read and accepted all of the following explanations regarding the removal of a laptop from the Pickler Memorial Library. Support for the laptop is provided by e-mail to firstname.lastname@example.org with help from IT services. The duration of this loan is between the date of issue and the date indicated above in the document. Laptops must be returned before or on this date to avoid penalties (see fines and penalties). Employees are cordially advised to request a further extension of their long-term loan term before the next application deadline, and some loans will be automatically renewed by your manager. Whether your application has been approved or not, you must return the laptop to IT Services Help Desk so that we can run software updates, change security settings, check for damage, and update hardware if necessary. I agree to take full responsibility for the laptop and its peripherals while getting examined. I understand that by borrowing this laptop, I am responsible for its return. I agree never to leave the computer unattended or in the care of another. If the laptop is stolen while it is on loan to me, I am responsible for the total cost of replacing the device. Accessories such as mice, adapters, cables, docking stations, external displays and other peripherals are not included in the delivery of a laptop and are the responsibility of the department.
